Sharma,<\/strong> Former Dean, Rajiv Gandhi Govt Post-Graduate Ayurvedic College Paprola- Himachal Pradesh<\/p>\n<p><strong>Definition:<\/strong><\/p>\n<p>Jara is state of becoming \u201cKsheen\u201d (debilitated), physically and psychologically weak as a result of old age usually after 60 years of life. The word geriatrics too is derived from Latin word \u201cgeras\u201d meaning \u201cto grow old\u201d. Aging is described as a progressive loss of adaptability with passage of time so that individual is less and less able to react abruptly to challenges from the external or internal environment. with time the individual becomes progressively more frail and in need of increasing support to maintain his or her autonomy.<\/p>\n<p><strong>Synonms:<\/strong><\/p>\n<p>The word jara has few synonyms like:<br \/>\nJayas (that which has grown),<br \/>\nVarshias (fully evolved or matured),<br \/>\nJyeshatha (that which is older in age),<br \/>\nJarishth (that which is many years old),<br \/>\nother synonyms used for old age are:<br \/>\nJaran, Jarati, Jaraut, Bridh, Nishaphala, Gataartava, Pilankani, Senescence, Geriatric, etc.<\/p>\n<p><strong>Dacade wise aging:<\/strong>According to Shrangdhar one attains decadewise aging as below.<\/p>\n<p>1. Balayam &#8211; childhood<br \/>\n2. Vriddhi \u2013 growth<br \/>\n3. Chhavi &#8211; luster<br \/>\n4. Medha \u2013 sharpness in perception<br \/>\n5. Twak &#8211; skin and appendages<br \/>\n6. Drishti \u2013 visual acuity<br \/>\n7. Shukra &#8211; fertility<br \/>\n8. Vikram \u2013 valor and courage<br \/>\n9. Vuddhi &#8211; cumulative intellect<br \/>\n10. Karamindri &#8211; physical capacities<\/p>\n<p>Finally there is loss of ultimate that is functions of mind and life sustenance.<\/p>\n<p><strong>Jara hetu ( Aetiopathogenesis of aging):<\/strong><\/p>\n<p>Kalasya parinam \u2013 swabhawaja or essential by Charak<br \/>\nOther ayurvedic literature mentions few specific factors for jara esp Aakaljara<br \/>\n1. Pantha \u2013 excessive travelling or physical activities<br \/>\n2. Sheetam \u2013 taking unwholesome or chilled food<br \/>\n3. Having sexual desires for older women or to remain sexually active even after onset of oldage.<br \/>\n4. Kaddana : taking tubers in excess in food.<br \/>\n5. Manaspratikoolta; to remain under continuous mental stress.<\/p>\n<p>Charak in description of Rasayana, describes that indulgence in \u201cgramya ahara\u201d leads to agnimandya, vataprakopa and dhatukshaya. Thus it is important factor for premature aging<\/p>\n<p>Two major factors responsible for aging are:<br \/>\n1. Srotorodha<br \/>\n2. Agnimandata.<\/p>\n<p><strong>What the modern science concludes regarding factors of aging:<\/strong><\/p>\n<p>1. D.N.A Cross linking theory : failure to manufacture essential enzymes\/proteins.<br \/>\n2. Random Damage Theory- free radical damage.<br \/>\n3. Somatic Mutation Theory- death in groups<br \/>\n4. Pacemaker theory or organ system : decay of endocrinal and immunological potentials<br \/>\n5. Death hormone<br \/>\nAging is gene\/environmental controlled phenomenon.<\/p>\n<p>Onset of jara is accompanied by: Dosha kshaya, Dhatu kshaya, Mala kshaya, Ooja kshaya and Mano karya kshaya. That is; deterioration of shareerika, vachika and manasika functions. All these are reflected in features of aging.<\/p>\n<p><strong>Major metabolic changes in geriatric patients:<\/strong><\/p>\n<p>-Presence of inclusion bodies in cells like metabolic debris, lipids, calcium, cholesterol<br \/>\n&#8211; intracellular sodium increases and potassium falls.<br \/>\n&#8211; ph and intracellular water contents decreases.<br \/>\n&#8211; total body water decreases<br \/>\n&#8211; fat accumulates in body leading to fall in specific gravity of body from 1060 to even 1036<br \/>\n&#8211; cellular metabolism suffers due to altered dna, rna and enzyme systems<br \/>\n&#8211; oxygen uptake and utilization decreases<br \/>\n&#8211; body temperature falls<\/p>\n<p><strong>Features of jara:<\/strong><br \/>\n&#8211; Sharirshaktiheena- regression of physical capacities<br \/>\n&#8211; Smiritinasha \u2013 diminishing memory<br \/>\n&#8211; Manasik glani \u2013 loss of cheerfulness and alertness<br \/>\n&#8211; Balinam &#8211; appearance of wrinkles<br \/>\n&#8211; Palitya \u2013 greying of hair<br \/>\n&#8211; Dantashaithilya \u2013 loosening of denture<br \/>\n&#8211; Swabhava vaipparya \u2013 change in personality components.<br \/>\n&#8211; Kasa swasa pravritti \u2013 proneness to repeated coughs and dyspnoea on effort<br \/>\n&#8211; Sarvakriya asamarthata ; loss of physical, perceptual and mental faculties<\/p>\n<p>Features of vatavriddhi are found as permanent features in old e.g.<br \/>\n-Vakayaparushta<br \/>\n-Karshya<br \/>\n-Gatrasfurana<br \/>\n-Nidranasho<br \/>\n-Alapbalatvam<br \/>\n-Ghadvirchissaya.<\/p>\n<p><strong>Old people show features of dosha and dhatu as well as mala kshaya:.<\/strong><\/p>\n<p>The important among them are:<br \/>\nMandachesta \u2013 slowness of physical and mental activities<br \/>\nAapraharsh \u2013 unsatisfying nature<br \/>\nMudhasangyata &#8211; slowness of perception and resolution<br \/>\nNishprabhava &#8211; loss of lusture<br \/>\nRookshta &#8211; dryness and wasting<br \/>\nSandhisathalya &#8211; unstability of joints<br \/>\nManoshunyta &#8211; lack of ideation<br \/>\nShramm &#8211; easy fatigability<br \/>\nShirashaithilya &#8211; prominant and tortuous blood vessals<br \/>\nSfeeg, greeva, udar shuskata : wasting of muscles<br \/>\nAsthi shool \u2013 degenerative bone disease<br \/>\nKlaivya \u2013 erectile dysfunctions and sterility.<\/p>\n<p><strong>Features of jara attributable to ooja kshaya:<\/strong><\/p>\n<p>Durbalata- general debility<br \/>\nAabeekashanam \u2013 intaking of small quantity of food.<br \/>\nVyathitendriya \u2013 ailments and diminished capacities of indriyas.<br \/>\nDuschaya \u2013 lack of lusture<br \/>\nDurmana \u2013 decreased mental capacities<br \/>\nRookshta \u2013 dryness of skin<br \/>\nFeatures of aging are due to anatomical and physiological regressions in body over the years. It is at physical metabolic and psychological level and once onset occurs it is progressive. Every system contributes to these features. Only when it hampers the quality of life , threatens life it is usually appreciated.Physical incapacities are more complained and attended than mental.<\/p>\n<p><strong>Selected age related changes and their consequences:<\/strong><\/p>\n<p>General \u2013 increased body fat and decreased body water leads to obesity, anorexia<br \/>\nEyes \u2013 presbiopia and lens opacity \u2013 blindness.creased testosterone, oestrogen \u2013 decreased libido, impotence, sterlity<br \/>\nRespiratory \u2013 decreased lung elasticity and increased lung stiffness \u2013 dyspnoea and hypoxia<br \/>\nCardiovascular- increased arteriosclerosis, decreased autonomic responsiveness \u2013 heart failure, lowered tissue profusion, heart block<br \/>\nGIT- decreased liver function \u2013 lowered appetite, portal ht, decreased gastric secretions \u2013 poor digestion, anaemia decreased colonic motality \u2013 constipation and impaction of stools.<br \/>\nHaematological and immune system \u2013 anaemia, lowered immunity, autoimmune damage<br \/>\nGenito urinary \u2013 increased frequency, retention, incontinency, BPH.<br \/>\nMuskuloskeletal \u2013 power power, tone , fracture, incordination.<br \/>\nNervous system \u2013 dementia, delerium and depression.tremers, ataxia.<\/p>\n<p><strong>Aging and mind functioning:<\/strong><\/p>\n<p>Major change is loss of neurons leading to dementia resulting- loss of memory, deterioration of intellect, change in behavior, change in personality, apathy<br \/>\nmood instability, alzheimer\u2019s disease leads to early onset of the features with fast progression, mind functioning deterioration is accompanied by proportianate bodly functioning deterioration<\/p>\n<p><strong>Why sudden interest in geriatrics?<\/strong><\/p>\n<p>Economic development of world with better food, better living conditions, better health care there is demographic transition. Further low birth and death rate has lowered younger and increased older population. Average life expectency over 60 years with<br \/>\nhigh geriatric load and high geriatric care need both domiciliary and hospital<\/p>\n<p><strong>Common geriatric ailments and remedies:<\/strong><\/p>\n<p>Cataract \u2013IOL.<br \/>\nDenture loss \u2013 artificial denture<br \/>\nHearing loss \u2013 hearing aid<br \/>\nOsteoarthritis \u2013 joint replacement<br \/>\nAtaxia \u2013 prosthetic support<br \/>\nBPH \u2013 TUR.<br \/>\nBut, what for brain, mind, autonomic and voluntary control over activities ?<br \/>\nExtended physical life may be vegetative in nature with low quality of life without<br \/>\nEmotional component, memory, insight, gross ataxia, incontinence, unproductively.<br \/>\nThe need is to extend life of mental or neurological components along with physical and other biological components of geriatric life for quality of life which gives<\/p>\n<p>Physical wellbeing<br \/>\nMental wellbeing<br \/>\nSpiritual wellbeing<br \/>\nSocial wellbeing<br \/>\nfor biological wellbeing ; preventive, curative, palliative as well supportive care is needed<\/p>\n<p><strong>Geriatric care through ayurveda:<\/strong><\/p>\n<p>Physical care: through proper swasthvritta, dincharya, ritucharya, yoga,<br \/>\nBiological care : through required balanced diet, vitamins, essential minerals, specific drugs and rasayanas like ashwagandha, chyavanprash, amritprash, shilajeet, medhya rasayana, pranakameeya and other ayushyakameeya rasayana.<br \/>\nMental and spiritual care: mangal, pranipatgamanam, sadvritta, aachar rasayana, aparigrha, family support<br \/>\nSocial care : By society and government through economic support, old age care home, free travelling and health insurance, legal support, terminal care. With ever falling child mortality rate and death date the number of geriatric patients and hence need of geriatric care is being felt. Of various aspects of geriatric care physical and mental care is most important to assure health and productivity in individuals till fag end of life. This will assure minimum hospitalization of geriatric patients and there by prevent additional load on hospital facilities which are already overflowing with uncontrolled epidemics of Communicable and non communicable ailments. State at its level has to assure social care, legal care, financial care of geriatric patients.
